Cancellation request form

To request a cancellation of your swim school enrolment, please complete the form below. Please note that cancellations submitted are not confirmed until you have been contacted by LPAC swim school.

Please review the LPAC swim school terms and conditions prior to submitting your cancellation request. Cancellation requests must be submitted a minimum of two (2) weeks prior to the requested cancellation date.

Please note a medical certificate must be provided within seven (7) days of the child's absence from lessons.
Do you hold a LTS Inner West Fitness Pass membership?* - required

Please note if you are currently receiving the discounted Inner West Fitness Pass membership for Learn to Swim families, a minimum commitment period of 2 months applies to this membership.

If you cancel your child's Learn to Swim enrolment, your Inner West Fitness Pass membership will be converted to the full rate following confirmation of your Learn to Swim enrolment cancellation.

Cancellations are not confirmed until you have been contacted by LPAC swim school. Cancellations are subject to our cancellation policy guidelines.
